For New Patients

New PatientsWhen coming to any Community Health Centers, Inc. Service site for the first time, please register before receiving services.

 

  • Your photo ID is required.

  • Your Social Security card is required. 

  • Bring any insurance cards, if applicable.

  • Please arrive 30 minutes prior to your appointment to register.

  • Registration also includes giving consent for your care and making arrangements to pay for your services. (Our providers may not treat you without your consent or the consent of a legal guardian, when applicable). Registration is updated annually. All persons wishing to use any Center service, except WIC, must register as users of the Center by presenting income verification before services may be rendered. Hours for registration are:

    •  Monday, Wednesday, Thursday: 8:30 a.m. to 4:30 p.m.
    •  Tuesday: 8:30 a.m. to 6:30 p.m.
    •  Friday: 10:00 a.m. to 4:30 p.m.

Registration Requirements:
Documentation of eligibility for sliding fee schedule is verified by presenting one of the following:

    • W-2 Form
    • Income Tax Return
    • Letter from employer on company letterhead.
    • Pay check stub
    • DHS Eligibility Statement

    No one is turned away due to inability to pay.

How to Pay

How to PayCommunity Health Centers, Inc. charges standard fees for medical, dental and behavioral health services. Please be prepared to pay a minimum fee upon arrival (similar to a co-payment). The additional fees accrued will be billed to you based upon your appropriate sliding fee schedule.

  • Dental Office Visit: $ 30.00
  • X-Rays (Dental and/or Medical): $ 20.00 per film
  • Medical Office Visit: $ 20.00 

Computerized statements are sent monthly. You may pay by cash, check, money order, MasterCard/Visa, or through your insurance. At your request, the Billing Department will be glad to work out a convenient payment plan. Standard collection procedures will be implemented for all past due accounts.

We collect annual income information so that we are able to determine a sliding fee scale in the event that your insurance does not cover the full amount. Please be aware that non-covered services or insurance rejections due to non-eligibility will be transferred to self-pay.

Calling Us After Hours

Call after hoursA Community Health Centers, Inc. provider is available to talk with you after our offices close and on weekends and holidays.  Your call will be forwarded to our answering service that will contact a provider from our staff. Simply, call our main office number 405-769-3301.

After hours medical services are limited to phone counseling, medicine prescriptions and, if needed, a referral to a hospital emergency room.

Hospital Care

If you need hospital care, Community Health Centers, Inc. Services staff will refer you to a local hospital. Tell the hospital staff that you are a CHCI patient so they will send your medical information to us. We include this material in your medical records to better serve you.

Community Health Centers, Inc. is not responsible for any healthcare services you receive outside our system.

Prescription Refill Line

Prescription RefillFor Prescription refills call 405-769-3301 extension 5607. We will need your first and last name (please spell difficult to pronounce names), your date of birth, your prescription number and the name of your prescription.

  • If you have refills for that prescription, they will be ready the same day.
  • If your prescription bottle has "call doctor" or is expired, please allow 24-48 hours for authorization from your provider.

Transportation

Transportation

    Courtesy transportation is available to transport registered clients to and from the Center. Clients needing transportation must reside within the Center's transportation boundaries and have no other means of getting to the Center. 

    The Center’s transportation boundaries are: West - Sooner Road; East - Choctaw Road; North - NE 122nd Road; South - Reno.

    Transportation service is available to clients with scheduled appointments between 9:00 a.m. and 2:00 p.m. Monday through Friday.

If you need transportation to and from your appointment, please contact advise your operator when you make your appointment or call at least one day prior to your appointment to be placed on the transportation schedule

If you call to cancel your appointment, also ask to be removed from the transportation schedule.

Be ready when the driver arrives at your home (the driver will not wait long since s/he must adhere to the transportation schedule).
